How to use the self care portal

Advice

The self care portal is a tool that you can use to complete a number of health care tasks. This might include learning about a new health condition that your have been diagnosed with, completing a medication review or making a treatment decision.

The portal is designed to help you and you therefor do not have to use it. If you would prefer to book an appointment with your health care professional please contact your surgery in the usual way.

Your health care team will be able to let you know what section of the portal you need to use and when.

When you are ready…

  1. Click PORTAL LIBRARY from the home page.
  2. Find the Click category that you need on the protal library page and openĀ  by clicking START
  3. Find the condition that you need and click the image to open.
  4. Watch the video.
  5. Read the advice and open the links.
  6. Read all of the FAQ.
  7. Complete the online form and click SUBMIT when ready.

Your health care professional will review your form and action accordingly.
